Legislative basis. Art. Art. 9, 10.1 of the Federal Law "On Personal Data" from 27.07.2006 № 152-FZ, Article 13 of the Federal Law of 21.11.2011 № 323-FZ "On the basis of public health in the Russian Federation", Roskomnadzor Order of 24.02.2021 № 18 "On approval of the content of consent to process personal data allowed to the subject of personal data for distribution".
In accordance with the requirements of Article 10.1 of the Federal Law "On Personal Data" № 152-FZ and Order of Roskomnadzor from 24.02.2021 № 18 "On Approval of the requirements for the content of consent to the processing of personal data, authorized by the subject of personal data for distribution", I confirm my consent to the processing of personal data by the operator, namely: Clinic of aesthetic medicine "DEGA" (Dega LLC, INN 7816639651/OGRN 1177847154396; Legal address: 190121, St. Petersburg, Rabochiy pereulok, 3, LITERA A, website: https://en.dega-clinic.com).
The purpose of the processing of personal data - making an appointment with a specialist, as well as the implementation of the rights and freedoms of man and citizen in the processing of his personal data, including the protection of the rights to privacy, personal and family secrets.
To make an appointment with a specialist, by filling in the electronic form on the Operator's website: dega-clinic.com, I give my consent to the processing of personal data, namely: phone number, first name, last name.
I grant the Operator the right to collect, systematize, accumulate, store, update, change, use, transfer, depersonalize, block, destroy these personal data, as well as other actions related to the processing of my personal data.
When processing personal data, the operator undertakes to take necessary legal, organizational and technical measures or to ensure their adoption to protect personal data from unauthorized or accidental access, destruction, modification, blocking, copying, provision, distribution of personal data, as well as other unlawful actions in relation to personal data.
It is clear to me that I have the right to establish a list and conditions for prohibiting the processing of personal data and to inform the Operator of these categories and conditions.
The text of this consent is stored electronically in a database and/or on paper and confirms the fact of consent to the processing and transfer of said personal data in accordance with the above provisions.
I reserve the right to withdraw my consent. If I receive my written request to withdraw my consent to the processing of personal data, the Operator is obliged to stop processing it. I confirm that by giving such consent, I am acting of my own free will and in my own interest.
The term of the Consent is from the date of transfer of personal data until I make the decision to enter into a contractual relationship with the clinic "DEGA" or until I revoke the Consent.
I hereby confirm that the personal data submitted by me to the Clinic of aesthetic medicine "DEGA" is reliable and can be processed by the Operator for the specified purposes. I confirm that by giving such consent I am acting of my own free will and in my own interest.
