WOULD YOU LIKE TO HAVE A RHINOSEPTOPLASTY?

Unlike rhinoplasty, which is more often performed for aesthetic reasons, the correction of the nasal septum, called rhinoseptoplasty, is more often performed for medical reasons. A deviated septum is found in many people, some of them are congenital and others acquire it over the course of life due to injury or disease. Most people have a slight curvature, it does not affect the outside of the nose and does not cause discomfort. But with a strong deformity nasal breathing suffers, often there are problems of the sinuses, chronic pathologies of the nasopharynx. This requires correction for medical indications.

PECULIARITIES OF RHINOSEPTOPLASTY

The surgery is considered to be a serious surgical procedure that requires the skill of a plastic surgeon or ENT doctor. The surgery is performed not so much to create a straight shape of the nose, as to restore proper breathing through the nostrils. Rhinoplasty and septoplasty are often combined in one procedure.

The nasal septum is a plate formed by bone and cartilage, and it divides the nose into left and right halves. In most cases people have a congenital asymmetry of the skull structures, so the septum is not perfectly flat, it may be tortuous or deviated to one side. If it does not interfere with breathing and does not provoke frequent colds, no treatment is necessary. But if there is a significant difficulty in breathing, rhinoplasty for a deviated septum is necessary.

Today, septorinoplasty, which eliminates the deviation of the nasal septum, is one of the most complex procedures in facial plastic surgery, and it is often prescribed in the treatment of problems of the ENT organs and the visual analyzer. During the operation, you can perform a plastic nose septum and correct its shape and size, but because of this will be higher the final price of the intervention.

DESCRIPTION

All indications for nasal rhinoseptoplasty can be divided into two groups: aesthetic, when there is no problem with breathing, and medical. These include:

  • A hump in the back of the nose, which can be of congenital or post-traumatic origin. The bumps can be symmetrical or irregular in shape. After surgery, this defect disappears;
  • A thickened bridge of the nose, which can occur in utero or after severe trauma. Bone dissection and septal resurfacing provide the best results;
  • saddle-shaped nose with recessed back and pointed upturned tip;
  • Curvature of the nose sideways with a deformity of the septum;
  • too wide or large nose, exceeding one third of the size of the face;
  • Abnormal shape of the nasal tip - hooked, bifurcation, thickening, flattening against the background of a deviated septum;
  • too thick, wide wings of the nose, disproportionality of the nostrils, their deformity;
  • severe bending or waviness of the septum, which is hardly visible externally, but sharply impairs breathing.

CONTRAINDICATIONS

As with any intervention, when performing rhinoseptoplasty in a clinic in St. Petersburg, there are a number of contraindications that must be considered when planning the operation. Thus, the operation does not take place before the age of 18 years, with the exception of serious injuries, which require immediate intervention. Extremely cautious about the operation after 40 years, when tissue regeneration is slowed, increasing the risk of complications. In addition, the following are also prohibited:

  • pregnancy and lactation;
  • abnormalities of the cardiovascular system (especially circulatory insufficiency, hypertension, heart defects);
  • serious kidney or liver disorders;
  • the presence of diabetes mellitus, which hinders tissue regeneration and impairs blood circulation;
  • clotting system disorders;
  • purulent processes of the skin and underlying tissues at the surgical site;
  • any type of tumor localized in different parts of the body;
  • acute infections, febrile states;
  • mental disorders.

SEASONS

The cost of rhinoplasty depends on the way it is performed and the extent of the intervention. It is performed either by a closed method, which is more gentle, but involves a smaller volume of tissue, or by an open method, when significant defects need to be corrected.

Closed rhinoseptoplasty is performed through one or more incisions in a shallow depth of the nasal mucosa. The doctor separates those parts of the skin that make up the base of the nose. Then the surgeon manipulates the cartilage, changing its shape by cutting off unnecessary parts or adding missing parts. If you need to narrow the nose, with its back cartilage is removed, the bone tissue is removed with a chisel. After the desired shape is obtained, the skin is returned in its place and fixed with sutures. Simultaneously with the correction of the external shape of the nose, the doctor evens the nasal septum, excising it in the area of defects and putting it in its proper place.

Open rhinoseptoplasty is used for reconstructive and repeated interventions. Incisions are made not only on the inner surface of the nasal septum, but also on the columella. It is also used when it is necessary to increase the volume deficit of soft tissues, a sharp deformation of the septum, to correct the consequences of severe trauma, abnormally grown tissues, to give the nose a perfect shape.


The intervention is performed by one of the selected methods under local or general anesthesia. The duration of the operation usually does not exceed 2 hours. A month before surgery you need to give up smoking, a couple of weeks before surgery blood thinning medications are canceled, other medications are discussed with the doctor. Specialist discusses with the patient how much rhinoseptoplasty of the nose costs, and appoints the date of intervention, determines the amount of preparatory activities.

It is necessary to undergo a full examination to rule out possible complications during anesthesia and the intervention itself. A full list of blood tests, coagulogram and photofluorography, ECG are required, as well as a consultation with a therapist and an anesthesiologist.

Before the surgery, the plastic surgeon holds a second consultation, where the doctor measures the parameters of the nose and relate them to the other elements of the face. He also performs a simulation of the result on the computer and discusses it with the patient.

The correction of the nasal septum is performed under general anesthesia, which is included in the price of the intervention. The doctor can perform only the correction of the septum without affecting the other structures (functional plasty), or perform a reconstructive surgery - a radical intervention on the septum and the structures of the nose. Based on the amount of intervention, the technique is selected - closed or open plasty.

Together with anesthesia and out of it the operation lasts up to 3-4 hours, after its completion the doctor puts special turundas with hemostatic compositions into the nasal passages, the back of the nose is fixed with a plaster splint.

REABILITATION

On average, the first rehabilitation period with gradual elimination of edema of internal structures and hematomas, and healing of sutures lasts up to two weeks. In case of open type of intervention it lengthens by another week.

The doctor removes the plaster splint after two weeks, the splints are removed at the same time. While they are inside the nasal passages, full nasal hygiene with careful washing with saline solutions is necessary. Nasal turundae are removed from the nose by the end of the second day, swelling subsides by about 4 day. During the rehabilitation period, you should not wear glasses, take hot baths, visit bathhouses and saunas.

If the deformity is severe, nasal breathing suffers, nasal sinus problems and chronic pathologies of the nasopharynx often occur. This requires correction on medical grounds.
